Instructions for updating Conflict of Interest (COI) attestations.

  1. Access the SIUSOM Website: https://coi.siumed.edu/.
  2. Log-in using your SIUSOM username and password (same as email).
  3. In the bottom right corner, enter your SIUSOM Employee ID Number, without leading zeros.
  4. Complete the Attestations by selecting the Research and CME Reporting tab.
  5. Scroll down on the Affirmations page, and in the lowest green box, below the purple text, CHECK the first box located in the bottom section
    -For public health service (PHS) funding: "I am submitting this attestation to: the ADRFA Office of Grants & Contracts to comply with PHS reporting requirements for PHS-funded research".
    -For NON-PHS funding: "I am submitting this attestation to: the ADRFA Office of Grants & Contracts to comply with reporting requirements for Other Sponsored Programs".
  6. Enter the PI's name, Title of the Application, Funding Agency. If you are the PI, enter the Funding Agency's Assignment number.
  7. Ensure the ADRFA Office of Grants & Contracts box is checked in the lower area. It should automatically select that box, if you choose the ADRFA options in the first area.
  8. Finally, choose the Submit Attestation button.
