
CENTRAL RESEARCH COMMITTEE APPLICATION
CHECKLIST
DIRECTIONS: ALL of the information requested must be supplied . Electronic submission is required. Electronically submitted proposals will be posted for reviewers on a secure website on the ADRAF's site. Electronic submissions need to be sent to Nancy Austin, at naustin@siumed.edu . The original and four hard copies need to be sent to mailcode 9616 or delivered to room 2073, 801 N. Rutledge.
The original and four (4) hard copies
of the entire application including the first page reviewed and
signed by the principal investigator, any co-investigators, and department chairman
and submitted to the Office of the Associate Dean for Research and Faculty Affairs,
801 North Rutledge, Room 2073 (mail code 9616). The department chairman's signature
will indicate that the application has been reviewed for scientific merit and
availability of facilities and personnel by the chairman or his/her designate
and found to be meritorious of CRC support.
Committee Approvals (if applicable): Make correct
entries on the Application for Central Research Committee Award form.
- Springfield Committee for Research Involving Human
Subjects
- Radiological Control Committee
- Laboratory Animal Care and Use Committee
- Institutional Biosafety Committee
Five (5) copies of each publication pertinent
to this application listed on Page 6 of the Application for Central Research
Committee Award form.
One (1) copy of the abstract for each
active and pending grant listed under "Other Support" on Page 7 of the Application
for Central Research Committee Award form.
Five (5) sets of graphs, diagrams, tables,
and charts that are submitted as appendix material.
One (1) copy of the Lay Research Summary.
The original and four (4) hard copies
of the Annual Progress Report submitted, per instructions, if
you are currently funded by the CRC and are requesting support
for a new project.
NOTE: If you are requesting continued
funding for the same project, you need not submit an Application
for Central Research Committee Award.; submit just original and four
(4) copies of Annual Progress Report, budget, budget justification,
and one (1) copy of Lay Research Summary.
