Please plan on bringing your insurance card to your first appointment. Co-payments are expected at the time of service unless other arrangements are made.If you have any questions please call patient billing at 217-545-4966.
New patients should plan on bringing copies of any previous X-rays, CAT Scans, PET Scans, Pulmonary Function Studies or other tests with them to their first appointment, as well as a list of all medications you are taking - including all vitamins and other supplements.
