SIU Carbondale Family Medicine is proud to be the first health care center in Southern Illinois to be certified as a Patient Centered Medical Home. A medical home is a concept designed to improve a patient's health care by providing a place where a patient has an on-going relationship with a health care provider, who monitors, directs and coordinates that patient's care. The focus is of care is not just on acute problems but on the overall health of the patient and their family.
