Applying for Credit
If you are interested in applying for credit for a CME activity or Regularly Scheduled Series, please contact the Office of CME at 217-545-7711 to determine if the activity meets the criteria for SIU School of Medicine sponsorship. You may then complete the application for credit.

In order to identify and resolve all potential conflicts of interests we ask that CME presenters and planners complete the Disclosure Form. For your convenience it is available as a fillable pdf document, editable on your computer. Please submit it as an attachment to an email addressed to Laura Worrall (lworrall@siumed.edu) at least 1 month prior to the activity.It is recommended that you save a copy for any modifications that you may need to make for future presentations.
