For every appointment, please check in at the reception desk when you arrive.

- Your payment/copayment
Since we operate by appointment only, please call us at 217-872-3800 to schedule, cancel or reschedule an appointment. If you are unable to keep your appointment please notify us 2 hours prior to your scheduled appointment time. This courtesy allows us to provide service to others.
After repeated broken appointments without being notified, we may discontinue your service so that others may be better served.
Billing, Insurance, and Collections
We accept most major insurances, including Medicare and Medicaid, but please contact your insurance company to determine if we accept your specific plan.
Payment is expected at the time of service; this includes copayments that insurance companies require us to collect at each visit.
For your convenience payments can be made in this office by cash, check, Visa, MasterCard or Discover.
A personal payment is required monthly on outstanding accounts. Delinquent accounts will be turned over to a collection agency.

Please contact your pharmacy to request a refill on your medications and they will contact us for approval.
A 48-hour notice is required for medication refills.
If you have questions about your medication refills, the SIU nurse may be contacted between 8:30am and 4:00pm for assistance.
Authorization for refills is NOT available during non-office hours.
Controlled substance refills will not be completed on Fridays.

Urgent problem after the clinic closed?
The SIU on-call physician may be reached (217-872-3800) after hours, 7 days a week, for urgent medical problems. You can expect a call back within 20 minutes.
If you are directed to go to the DMH Emergency Room, please let the ER know that you are an SIU patient when you arrive.
