WHITE CONSORTIUM
Thank you
for your interest in the Central Illinois HIV CARE Consortium. The
Consortium is a program funded by the Illinois Department of Public
Health through the Reauthorized Ryan White Comprehensive AIDS Resources
Emergency (CARE) Act Amendments of 1996.
SIU School of Medicine
has been designated as the lead agency to administer the activities
of the Consortium. The Consortium consists of medical and social
service providers who are contracted to provide services to a diverse
group of individuals residing in a seventeen-county service region.
The purpose of the Consortium
is to support and facilitate the provision of coordinated and comprehensive
health and support services to people living with HIV/AIDS.

Eligibility Criteria
- Documented proof of
HIV or AIDS Status
- Resident of county
within Consortium service region
- Enrollment in case
management services
- Meet income guidelines

Services
Most
services have income eligibility requirements with the exception
of case management, nutrition and counseling.
