Pulmonary Consult Service

CONTACT INFORMATION/ORIENTATION

  1. Contact the Pulmonary Consult Fellow or the Pulmonary Administrative Office (545-0187) to find out where/when to report on the First Day of the rotation.
  2. Meet with the Attending Faculty on the First Day of the rotation for orientation and sign orientation attendance sheet.
  3. Meet with Amy Hoffman, RN (SIU Clinic Building – Room Lower Level – 545-5150) when attending your first pulmonary clinic for clinic orientation.

EDUCATIONAL PURPOSE

Residents will improve their competency in the evaluation and management of inpatients and outpatients with pulmonary diseases and related complaints.

Residents will provide consultation for inpatients and outpatients with pulmonary diseases and related complaints.

Residents will improve skills at interpreting chest x-rays, chest CT scans and other pulmonary imaging modalities.

Residents will improve their skills in interpreting ABGs and their knowledge of oxygen delivery systems.

Rounds:

Daily with attending on service—average two hours per day.

Clinics:

One half-day clinics per week with attending on service or other pulmonary faculty
Notify attending/fellow when you will have your continuity clinic

Chronic Obstructive Pulmonary Disease Cystic Fibrosis
Restrictive lung disease Occupational lung diseases
Asthma Lung abscess
Community-acquired pneumonia Pneumothorax
Tuberculosis Sleep apnea/sleep disorders
Fungal infection of the lungs Lung cancer
Hypersensitivity pneumonitis Sarcoidosis
Chronic cough, Dyspnea  

PROCEDURES

Thoracentesis
Chest tube insertion
Perform and interpret pulmonary function tests (observe PFT performance in SIU clinic and/or MMC PFT lab)

SERVICES

Evaluation and management of inpatients and outpatients with pulmonary diseases and related complaints.

Provide consultation for inpatients and outpatients with pulmonary diseases and related complaints.

READING LIST

All pulmonary chapters in Cecil’s or Harrison’s Textbook of Medicine

Other pulmonary medicine texts are available to check out in the Pulmonary Library D425 located within D434.

A variety of pulmonary medicine topics are covered in power point presentations on the pulmonary website.

PATHOLOGIC MATERIAL

Reviewed on a case by case basis with attending physician and pathologist where appropriate to patient care.

OTHER EDUCATIONAL RESOURCES

Become familiar with the following respiratory medicine topics by using education resources available in the Pulmonary Library and teaching sessions with attendings/fellows:

  • Obstructive Lung Diseases
  • Interstitial Lung Disease
  • Lung Neoplasms
  • Respiratory Tract Infections
  • Sleep-Related Breathing Disorders
  • Pleural and Chest Wall Disorders
  • Chest Imaging
  • Basic Spirometry (Performance & Interpretation)

METHOD OF EVALUATION

Faculty complete resident performance evaluation forms. Residents complete faculty and rotation evaluation forms. Complete required Mini-Clinical Evaluation Exercises (Mini-CEX): Respiratory Exam in PGY 1

Meet with attending faculty for face-to-face feedback during last week of the rotation.

LEVEL OF SUPERVISION BY FACULTY

Residents with little experience in the skills needed will observe and practice physical exam skills under direct supervision of attending physicians, while those with more experience will work with patients in concert with attending physicians.
