REFERRING A PATIENT
Cardiology
To refer a new
patient to the SIU/Prairie Cardiovascular Clinic, please call
our office at 217/545-0185.
In-patient consultation
is available at Memorial Medical Center through the SIU/Prairie
Cardiology service between the hours of 6:00 a.m. and 5:00 p.m
weekdays. Please be sure to indicate to the hospital staff that
you would like an SIU Cardiology consult or call 545-0185. For
inpatient consult requests between 5:00 p.m. and 6:00 a.m. and
weekends or holidays, please contact the Prairie Cardiovascular
Consultants physician on call at 788-0706.
Our Dermatology
Clinic takes patient referrals for all types of skin ailments. We
are located on the 2nd floor of the SIU Clinics Building at 751
N. Rutledge. To refer a patient to the SIU Dermatology Clinic, please
call 217/545-3821.
For outpatient services,
the office of the referring physician should contact the clinic
appointment desk at 545-0182 and state that they are making
an Endocrinology referral. Our clinic is located on the 2nd
floor of the SIU Clinics Building at 751 N. Rutledge.
In-patient consultations
are available at both Memorial Medical Center and St. John’s
Hospital by calling 217/545-0160 weekdays 8:00 to 4:30. For after
hours, have the hospital staff page the resident on call for Endocrinology.
Contact
the GI office at 545-0173 and provide the following information:

Has
the patient seen a GI doctor in the past? If yes, please provide
all old records.

Diagnosis/reason
for referral along with any and all tests, labs and procedures
done in relation to patients ailment

Most
recent H&P and medicine list
Once the
GI office receives all of the above information, they will make
the appointment based on clinic availability.
Appointments
will not be made for new patients unless a call is received from
the referring physician office.
If a patient
has been previously seen, but is presenting a new problem/diagnosis,
the patient will need a new referral to the GI office.
General
Internal Medicine
Outpatient:
Physicians in the Department of General Internal Medicine welcome
new adult patients seeking a primary care provider. To establish
care with one of our physicians contact our clinic at 217-545-0182.
We accept most insurance plans and care for uninsured patients.
The reception staff will assist you in making an appointment.
Inpatient:
The Department of General Internal Medicine also provides a Hospitalist
service at St. John’s Hospital and Memorial Medical Center.
If you are a physician requesting a transfer of your patient to
one of our Hospitalists, please contact the switchboard operator
at St. John’s (217-544-6464) or Memorial Medical Center
(217-788-3000). We, in turn, can enlist the appropriate subspecialty
help if needed.
The
referring physician's office should call the SIU Hematology/Oncology
office at 217/545-5817 with the appropriate patient information.
The
physician's office should then send the patient's records by mail
(preferred) to PO Box 19678, Springfield, IL, 62794, or with the
patient.
For
outpatient services, the office of the referring physician should
contact the Infectious Diseases nursing staff at 545-9537 or 545-9346
to explain the urgency of the referral.
For
outpatient services, the office of the referring physician should
contact the Medicine Psychiatry nursing staff at 545-8075 to explain
the urgency of the referral.
To refer
a patient to the CIKD outpatient clinic, or for an in-patient consultation,
please call our office at 544-5100.
For
outpatient services, the office of the referring physician should
contact the clinic appointment desk at 545-0182 and state that
they are making an Pulmonary referral. Our clinic is located
on the lower level of the SIU Clinics Building at 751 N. Rutledge.
In-patient
consultations are available at both Memorial Medical Center
and St. John’s Hospital by calling 217/545-0187 weekdays
8:00 to 4:30. For after hours, call 545-8000 to page the Pulmonary
fellow on the consult service.
For
outpatient services, the office of the referring physician should
contact the clinic appointment desk at 545-0182 and state that
they are making a referral.
For
inpatient/consult services, the referring physician should contact
the Rheumatology office at 545-0184 with all necessary patient
information.
