MINIMUM QUALIFICATIONS: |

1. M.D. or D.O.
degree
2. Completed an AOA
or ACGME approved residency in general and child and
adolescent psychiatry
3. Board eligible in
General Psychiatry and Child and Adolescent Psychiatry
4. Eligible for
licensure with the State of Illinois |
