Provide management of
established patients in the Memory Disorder clinics; Provide
education to patients and families to improve, promote and maintain
their health status through counseling and providing information on
disease process, self care practices, mind/body wellness and
positive life style choices |

MINIMUM QUALIFICATIONS: |
|
1. Certified Nurse
Practitioner degree from an accredited training program
2. Special interest
in Alzheimer Disease, other dementia, geriatrics or movement
disorders
3. Must be licensed
in the State of Illinois as a CNP |
