JOB SUMMARY: |

Work in a
collaborative practice with faculty physicians and other mid-levels
to provide care to patients of the Center; In addition,
opportunities will exist to teach and evaluate family medicine
residents in training; Major portion of time will be assigned to an
Acute Care (Same Day) Clinic and assist faculty in delivering care
to their patient panels |

MINIMUM QUALIFICATIONS
FOR PHYSICIAN ASSISTANT-CERTIFIED: |
|
1. Graduation from an
accredited Physician Assistant training program
2. Credentialing
through the National Commission on Certification of Physician
Assistants
3. Licensure in the
State of Illinois as a Physician Assistant-Certified
4. Experience in a
primary care or urgent care setting |

MINIMUM QUALIFICATIONS
CERTIFIED FAMILY NURSE PRACTITIONER: |

1. Master's degree
in Nursing
2. Certification
through the American Nurses Credentialing Center as an Advanced
Practice Nurse
3. Licensure in the
State of Illinois as a Certified Family Nurse Practitioner
4. Experience in a
primary care or urgent care setting |
