SIU Center for Family Medicine - Quincy, is conveniently located just a across the street from the main Blessing Hospital campus at 11th street.

The office is designed to promote comprehensive continuous family care. The staff are trained to understand that the function of the family unit has a significant impact on health and illness. To this end, psychological and social services are provided at SIU Center for Family Medicine - Quincy and instruction in counseling, interviewing and communication skills and techniques to enhance patient compliance occur at all levels. In this complete and congenial setting, the resident learns the team approach to family care, efficiently utilizing community resources, consultations, referrals and office resources.

Patient Check-In

Effective July 1, 2016, a photo ID is required upon arrival for your appointment.

When you arrive at our facility, please check in with a receptionist. As a new patient you will be greeted at the reception desk and asked to complete several forms to establish your record. In order to do this, please bring:

Photo Identification

  • Government issued ID
  • Employee ID
  • School ID

Insurance Information

  • Your insurance card
  • The address of your insurance company
  • Any Workers Compensation information, if it applies to you

Health Information

  • Past hospitalization and surgeries
  • Family medical history
  • All medicines being taken
  • A record of all immunizations

Please tell the receptionist at the time you register whether or not you wish to have previous medical records transferred. If so, you will be asked to complete a transfer form.

Confidentiality

The Faculty utilizes a closed circuit television system in a highly confidential manner as a part of resident training. You will be asked to sign a form consenting to the use of the system during your visit. If you have any concerns about this aspect of the program, please talk to a member of the staff.

Your care is strickly private. No information regarding your care will be given without permission, except when required by law.

Billing, Insurance, and Collections

We ask that you make payment at the time service is provided. Copays are expected at the time of service. Please bring all insurance, Medicare, and/or Medicaid cards with you. We will need copies of your insurance cards to place in your file. We will be happy to assist you in filing your insurance and secondary insurance claims. We participate in many managed care, PPO and HMO insurance plans. Please contact your carrier to be certain we paricipate in your plan. We offer discounted fees for patients who qualify.

For those patients who are self-pay or who have insurances that do not cover office visits, payment is due in full at time of service.
For your convenience, copayments and balances due can be made in our office. We will gladly accept cash, checks, Visa, MasterCard or Discover.

Financial Counselors are available to assist you by calling (217) 277-5760 or (217) 224-9484.

Our insurance policy requires a personal payment each month on outstanding accounts. In case of non-payment, patients are given adequate time and notice to settle their account, but our office strictly adheres to an established collection procedure. Delinquent accounts may be turned over to a collection agency.

Appointments

We primarily operate by appointment, but also offer walk-In appointments. Same day appointments are available. The patient support center will ask about the nature of your problem, in order to allow enough time for your appointment. In some cases, you may be asked to speak with a nurse who will determine appropriate services.

If you are unable to keep your appointment, or will arrive late, please call the office at least two hours in advance of your scheduled appointment at (217) 224-9484.

This courtesy allows us to provide service to other patients. The clinic has a "NO SHOW" policy for repeated broken appointments that have not been cancelled. After repeated failures to keep or cancel an appointment, patient may have their service changed to walk-in appointments only, so that others may be better served.

Medication Refills

Please contact the pharmacy of your choice for refills. Ask the pharmacy to contact us for physician approval of your refills. The SIU nurse may be contacted between 8:30 a.m. and 4:30 p.m. for refill assistance. A 48-hour notice for medication refills will be required. Authorization for refills is not available during non-office hours.

    Doctors of osteopathic medicine (D.O.’s) are physicians who are trained and licensed to prescribe medication, perform surgery and use osteopathic manipulation.

If you need to be hospitalized, we admit patients to Blessing Hospital. Our physicians will provide and coordinate your care during your stay in Blessing Hospital.

Emergency after the clinic is closed?

The SIU on-call physician may be reached at (855) 228-6768. SIU physicians are on-call 24 hours a day, 7 days a week, for our patients with an urgent medical problem. Generally the after-hours service does not refill prescriptions.

If you are directed to go to the Emergency Room, please let the ER know that you are an SIU patient and to notify an SIU physician that you have arrived.
