Best Work (Summary of Two Publications):

Contralateral Prophylactic Mastectomy Improves the Outcome of Selected Patients with Breast Cancer. Am J Surg 2000; 180:439-445.
This retrospective study was one of the first to examine the efficacy of contralateral prophylactic mastectomy (CPM) in women matched for age, stage, surgery, chemotherapy, and hormonal therapy. We found that the CPM group had a disease-free survival at 15 years of 55% versus 28% in the control group (p=0.01). The overall survival at 15 years was 64% versus 48% (p=0.26). Other relevant findings were that the control group, being matched to women who opted for CPM, had a higher than average rate of contralateral breast cancer (1.8%/year compared to historical rate of 0.7%/year), thus suggesting a profile for whom CPM may be beneficial. This profile included young age, multifocal primary tumor, 1st-degree relative with breast cancer, and dense breasts. Since this paper was published, there has been an actual rise in the percentage of patients opting for CPM, and this paper has been cited in several review articles.

Vitamin E Increases Biomarkers of Estrogen Stimulation When Taken With Tamoxifen. J Surg Res 2008 Apr 22. [Epub ahead of print]
This prospective study was presented at the most recent Academic Surgical Congress and represents translation of my previous bench research into clinical relevance. In cultured breast cancer cells, supplemental vitamin E abrogates the apoptotic response to tamoxifen. Because many breast cancer patients take supplemental vitamin E while taking tamoxifen, I studied whether there were biomarkers of tamoxifen resistance present when the subjects were taking supplemental vitamin E. I found that activated phospho-ERK and estrogen receptor staining increased with vitamin E and serum tamoxifen levels decreased with vitamin E supplementation. This study, while small, indicates the importance of recognizing complementary or alternative medicine as potential harm.
