Patient Expectations

• Your Co-pay is due at the time of service.  Your co-pay is a contracted agreement that you have with your insurance company.  Patients can be asked to reschedule their appointment if co-pay is not paid. 
  
  
• Self pay patients are expected to pay $75 at your initial visit and $25 at any additional visits. (Financial aid is available by calling the patient billing office at 545-4966.)

• Please be on time for your appointment.  Being late can cause a disruption of the clinic flow, which will require the patient to reschedule their appointment.

• 24 hour notice needs to be given to cancel or reschedule your appointment.  Patients can be terminated from seeing their physician if this is not followed.

• Work Comp needs prior approval.  If a patient presents with out giving prior notification to the office that this is work comp related they will need to reschedule the appointment. 
  This allows the clinic staff time to get approval for your initial visit.
