Surgical Skills Lab Utilization Form

• Programs must be booked at least one month prior to the scheduled date.
• A meeting must be scheduled with the Surgical Skills Lab coordinator to assist with the planning of your session.
• Access to the lab for set up prior to the program session is dependent on lab availability.
  

FEES FOR USE OF THE SURGICAL SKILLS LAB INCLUDE:

• Instruments, Equipment, Space and Lab Assistant
  
• An array of instrumentation and equipment is available for use.
  
• Nitrogen, surgical suction and power outlets are available at each of the 4 workstations.
  
• It is the responsibility of the organizing program to arrange for any specialty equipment and models required.
