About the Office of Community Care

Established in July 2019, the mission of the Office of Community Initiatives and Complex Care is to remove "complex" out of "complex care" and support programs that bring improved access, increase self-sufficiency, and increase healing for all. The office has multiple programs available to our community. All programs use motivational interviewing, harm reduction, and trauma-informed care concepts to decrease isolation and develop a trusting relationship with residents living in central and southern Illinois. Programs include community health workers, trauma informed-care services and training, and medical student hotspotting training. In addition, the team provides consulting to other hospitals, medical clinics, police departments, court systems, and schools to develop population health-directed programs. The office is comprised of community health workers, nurses, community health educators, licensed clinical social workers, and educators who, together, provide services across numerous counties in central and southern Illinois.
