
About the Office of Community Health Work
Established in July 2019, the mission of the Office of Community Health Work is to take the “complex” out of “complex care” by supporting programs that improve access, foster self-sufficiency, and promote healing for all. The office offers multiple programs to communities across central and southern Illinois, all rooted in motivational interviewing, harm reduction, and trauma-informed care to reduce isolation and build trust with residents.
Programs include community health worker services, trauma-informed care training and support, and medical student hotspotting experiences. The team also consults with hospitals, clinics, police departments, court systems, and schools to help create population health-focused initiatives. The office includes community health workers, nurses, health educators, licensed clinical social workers, and other professionals working together to deliver care and support across the region.
