Patient safety information
This document explains the institutional and accreditation requirements for resident participation in patient safety reporting, and outlines how to submit and track safety events across multiple affiliated hospitals. It emphasizes that all residents must submit at least one de-identified patient safety event during their first year.
This one-page card summarizes where residents should report safety events depending on the clinical site and provides immediate steps and contact numbers for needlestick exposures.
Use the correct reporting platform based on where the event occurred:
