    Longitudinal rotations

    Primary Care Continuity Clinic at the Quincy Family Practice Center (QFMR)
    The continuity clinic is the core of our sports medicine program.  It allows the sports medicine fellow to maintain much-needed primary care skills while learning musculoskeletal skills.  The continuity clinic will include patients of all ages and scope in family medicine.  The continuity care clinic will occur throughout the year of training and will occur on average two (2) half days a week during the year.  The sports medicine fellow will be supervised by faculty at the SIU QFMR.

    The Primary Care Sports Medicine experience 
    This experience will be provided through several clinics and/or locations and will provide the main primary care sports medicine experience for the sports medicine fellow.   

    • Orthopedic Walk-In Clinic: This training will occur longitudinally during fellowship.  The fellow will have the opportunity to evaluate acute and chronic musculoskeletal conditions and injuries for patients of all ages. This experience will expose the fellow to evaluation of acute and chronic musculoskeletal injuries and conditions, fracture evaluation and management, muscle and tendon tears, dislocations / subluxations, and appropriate surgical referral to specialists. 
    • Rural Primary Care Sports Medicine Clinic:  This will occur one half day a week as a longitudinal experience at Hannibal Clinic with a Primary Care Sports Medicine trained physician and a Pediatric Sports Medicine trained physician.  The sports medicine clinic cares for patients of all ages with musculoskeletal injuries and conditions.
    • MSK Ultrasound Training: This training will occur longitudinally during fellowship year.  The fellow will have the opportunity to work toward RMSK ultrasound certification during their training which requires the fellow to obtain 150 scans as well as attending CME specific to MSK US. The fellow may choose to purchase their own personal handheld ultrasound unit before or during their fellowship year. The fellow will be exposed to Point of Care ultrasound in primary care as well as opportunities to evaluate musculoskeletal conditions in the primary care clinic, the orthopedic walk in clinic, and during training room clinics at the local colleges. 
    • Athletic Training Room Rounds:  This experience is supervised by sports medicine faculty.  The fellow will attend Culver Stockton College and Quincy University Athletic Training Rooms at least once a week.  The sports medicine fellow will have contact with his/her team coach/athletic trainer.  Injuries seen at the clinic include musculoskeletal-related cases or problems related to exercise. There occasionally will be sports physicals and illness evaluations provided.  This experience allows for evaluation and rehabilitation of injuries with state-of-the art equipment.
    • Quincy University Athletic Training Room: Quincy University competes at Division II level and has an approximately 1,000 square ft. athletic training room to treat student athletes.  The clinic is limited to Quincy University student athletes.  The athlete will be evaluated in conjunction with one of the athletic trainers employed by QU.  
    • Culver Stockton College Athletic Training Room:  Culver Stockton College competes at NAIA level and has an on-site 1,100 square feet athletic training facility as well as an educational center with simulation rooms.  Student athletes will be evaluated at one of the sites in a separate exam room in conjunction with one of the athletic trainers employed by CSC.  Culver Stockton College also has a Masters Level Athletic Training Education Program which will allow the fellow an opportunity for teaching rounds and providing lectures for these students.

    Midwest Orthopedics, Hannibal, MO.  
    This experience will be coordinated by the sports medicine fellowship coordinator and the office manager at Midwest Orthopedics.  This experience allows the sports medicine fellow to observe state of the art musculoskeletal medicine in a small town.  The primary care sports medicine fellow will obtain casting skills, joint aspiration skills, and joint injection skills.  Midwest Orthopedics has a broad base of patients of all ages with varied orthopedic problems.  

    Blessing Health System Orthopedics and Sports Medicine, Quincy, IL.  

    • This experience will be coordinated by the sports medicine fellowship coordinator and the office manager at Blessing Orthopedics.  This experience allows the sports medicine fellow to observe and participate in care of patients in a multispecialty orthopedics office.  Exposure to a broad range of patient ages and orthopedic conditions.  
    • The fellow will obtain skills in casting and splinting, joint injection and aspiration, management and follow up process for orthopedic injuries and conditions.
    • The sports medicine fellow will have the opportunity to follow and learn from general, sports medicine, upper extremity, and trauma orthopedic surgeons.
    • The fellow will participate in weekly orthopedic CME presentations and round-table discussions with the orthopedic surgeon group where they will discuss current patients on the orthopedic schedule and hospital service. 

    Physical Therapy and Rehabilitation

    The sports medicine fellow will rotate with physical therapists and athletic trainers at Advance Physical Therapy, a multi-state neuromusculoskeletal and sports medicine rehab group.  The sports medicine fellow will observe and assist patient rehabilitation from both surgical and non-surgical musculoskeletal problems.  

    Game/Event Coverage 

    The sports medicine fellow will be assigned a number of teams in which she/he is to function as the team physician, in consultation with the sports medicine faculty.  Collegiate and high school teams will be followed at all times.  The teams will be from Quincy University’s varsity sports and Culver Stockton College’s varsity sports.  Palmyra Missouri High School football will also be covered by the sports medicine fellow under the direct supervision of a primary care sports medicine trained physician.   The sports medicine fellow will provide care for an even mixture of men and women involved in contact and non-contact sports.    

    Blocked rotations

    Quincy Family Medicine (QFM) Inpatient Service

    This experience is supervised by faculty of the QFMR.  When a patient is admitted to the hospital by the sports medicine fellow from the continuity clinic, game coverage or training room experience, s/he will be supervised by QFMR Faculty.   

    The Sports Medicine Fellow will be scheduled to do Inpatient Service for one block and will be on call during the resident retreat. If the fellow has specific interests in academics, extra time on the service and teaching rounds can be arranged. At no time is the Sports Medicine Fellow required to provide patient care or supervise the residents without supervision by an attending. 

    University of Missouri – Columbia (MIZZOU) Sports Medicine Experience  

    • The sports medicine fellow will be exposed to a tertiary care center and have ample opportunity to interact with orthopedic residents, family medicine residents and athletic trainers.  While continuing to have a clinic once a week at QFM to maintain patient continuity, the sports medicine fellow will spend five weeks (to be determined by the sports medicine fellowship coordinator and Dr. Patrick Smith’s office manager) in Columbia, Missouri.
    • The sports medicine fellow must make his/her own lodging arrangements while in Columbia and is given educational funds to help offset the cost.  
    • The sports medicine fellow will be given one day off a week during this time, most likely on Sunday.   Most of the rotation time will be spent at the Columbia Orthopedic Group which is a multi-specialty orthopedic physician group in the Columbia, Missouri area.
    • The sports medicine fellow will also have the opportunity to spend time with radiologists that have complete practices dedicated to musculoskeletal radiology and imaging.  This is determined by the fellow and their initiative in contacting the radiologists.
    • The fellow will also be exposed to at least one Division I athletic event.  Arrangements must be made by the fellow to attend one athletic event with the team orthopedic physician.  

    Podiatry Clinic  

    A block experience with approximately ten (10) half days will be spent in a podiatry clinic in Quincy, Illinois.  The fellow will evaluate forefoot pathology, orthotic fitting, preventative care, and evaluation of the diabetic foot.

    QMG Rheumatology Quincy, IL (elective) 

    A block experience which averages ten (10) half days will be spent with the preceptors who specialize in a rheumatology clinic.  The fellow will continue to have a clinic once a week at QFM to maintain patient continuity.

    Skill-based rotations

    ACSM Fellow Research Conference  

    The sports medicine fellow will receive a stipend to attend the American College of Sports Medicine Fellow Research Workshop, typically a weekend held in late July/early August.  This allows the fellow an opportunity to interact with fellows from all disciplines and gives them the tools to work on a research project if they so choose during their fellowship.

    AMSSM Conference  

    The sports medicine fellow will receive a stipend to attend this conference.  The sports medicine fellow should prepare a study, poster presentation, or free-standing paper.  This will allow the sports medicine fellow to present a scientific paper or case study to a national audience of their future peers.

    Moonlighting opportunity 

    Moonlighting experiences are completely voluntary and can be terminated by the SIU Fellowship Director if it is felt that it interferes with the sports medicine fellow’s duties.  It is strongly encouraged that the sports medicine fellow does NOT do moonlighting during the football season.

    Mass pre-participation and sports exam responsibilities

    This is supervised by sports medicine faculty.  As a community service the QFMR provides pre-participation physicals to the community each year without charge.  Physicals are completed in accordance with the monograph of joint sponsorship with the American Academy of Family Physicians, Academy of Orthopedic Surgeons, etc.  The sports medicine fellow will participate in all available sessions and coordinate the above.  

    Mass participation events

    • The sports medicine fellow will participate in and be involved in planning medical coverage, EMS coordination, etc. for one of the community’s mass participation events.  This could include but is not limited to:
    • The Quincy YMCA Annual Turkey Run, yearly on Thanksgiving; open to runners of all ages and skill levels.  
    • Quincy Medical Group Healthcare Foundation Bridge the Gap Run; Yearly, May;  available events are 5K run/walk, 10K run, half marathon, and kids fun run.  
    • The Chicago Marathon is an opportunity given to the sports medicine fellow.  October , yearly.  They will work as a member of the QFMR medical team providing medical coverage to the runners.
    • Currently the program is working on obtaining an opportunity to work at a professional event, i.e. NFL Combine, XFL game
