This document and checklist is a resource for providers/researchers that are approached by industry sponsors to perform research services. These activities range from consulting to full collaborative research projects.

The Southern Illinois University School of Medicine Adult Reconstruction Fellowship is a built around a mentorship model. The fellowship is organized in rotations in which you cover a single provider in all phases of patient care including pre-operative workup, surgery, post-operative recovery and clinical follow up. SPRINGFIELD, IL - Family physicians in nine Illinois communities are hosting students from Southern Illinois University School of Medicine through September 27.
